Please ask at reception to register for our Patient Access Service. You will be required to attend the surgery as we will ask for proof of ID
Please allow TWO working days for your REPEAT prescription to be processed IN SURGERY.
You must then give the chemist time to dispense your medication.
Please note that medication not on repeat can take longer and THREE days should be allowed

Unfortunately we are unable to take requests for prescriptions on the telephone, though you may fax your request on (01926) 851746.
Medicines Use Review
Your local pharmacist would like to invite you to have a Medicines Use Review (MUR) because we have noted that you are taking at least one of the following medicines:
Studies have shown that as many as half the patients that take these medicines, either do not take them correctly or may have problems with them that could even result in an unwanted hospital admission.
A Medicine Use Review (MUR) is not a full clinical review such as you would have with your doctor or nurse, and it is not intended to assess the appropriateness of prescribed medication. It is a structured review to support your medicines use, help you understand your medicines better, and to identify any problems you might be having and suggest possible solutions.
The review takes place at your local pharmacy at a time convenient to you.

If you are on regular medication your doctor may issue you with a repeat prescription. This will be reviewed from time to time. On the right-hand side of the prescription is a tear-off sheet giving details of all the repeat medication you are on. It is important that you use this slip to order further prescriptions.
Tick the items you require and either drop it into the box in reception or post it to us. Enclosing a stamped, addressed envelope if you wish us to return it to you.
Please note that while we offer a service whereby we send your prescription to the local Pharmacy we cannot guarantee this. We have on average over 250 requests per day therefore there may be an odd occasion where it is filed at the surgery by mistake. If you need an urgent supply over a Weekend/Bank Holiday then you should contact the "Out Of Hours Service". If you obtain an emergency supply from the pharmacy there will be a charge. This charge is non-refundable by the pharmacy or the surgery. It is up to you to check that the prescription has been received by the pharmacy.
