Repeat Prescriptions

If you are on regular medication your doctor may issue you with a repeat prescription. This will be reviewed from time to time. On the right-hand side of the prescription is a tear-off sheet giving details of all the repeat medication you are on. It is important that you use this slip to order further prescriptions.

Tick the items you require and either drop it into the box in reception or post it to us. Enclosing a stamped, addressed envelope if you wish us to return it to you.

Please note that while we offer a service whereby we send your prescription to the local Pharmacy we cannot guarantee this. We have on average over 250 requests per day therefore there may be an odd occasion where it is filed at the surgery by mistake. If you need an urgent supply over a Weekend/Bank Holiday then you should contact the "Out Of Hours Service". If you obtain an emergency supply from the pharmacy there will be a charge. This charge is non-refundable by the pharmacy or the surgery. It is up to you to check that the prescription has been received by the pharmacy.
