Sick Certificates

Sick Notes information for patients

If you have less than 7 days off work, you can self certify your sickness absence by completing form SC2 obtained from your employer, or SC1 (for self-employed), obtained from the DSS/Tax Office.  We also have a small number at the surgery.

If you are off work for more than seven days, you will need a doctor’s certificate. A Doctor can only certify your sickness if you have been seen at the surgery or if we have written documentation from a hospital letter confirming the reason for your absence from work.

If you need another sick note and have seen a Doctor regarding your illness or we have written documentation from a hospital letter confirming the reason for your absence from work, then the Doctor can issue another sick note, if appropriate. The doctor may need to speak to you before issuing a further note. 

If you require another sick note please speak to the reception staff. They wil then pass this message on to your GP.

Please note we can not pre-date a sick note.

If your employer requests a doctor’s certificate before the seven days, a charge will be made.
